How to Refer to HMR Australia
To refer a patient to HMR Australia, General Practitioners simply need to email the completed HMR referral form to admin@hmraustralia.com. Our team will then process the referral and coordinate the Home Medicines Review.
HMR Service Eligibility Criteria
To be eligible for a Home Medicines Review with HMR Australia, the patient must satisfy the following criteria:
• The Patient is a current Medicare or DVA cardholder or is a person who is eligible for a Medicare card.• The Patient is living in a community setting.
• The Patient is at risk of, or experiencing, medication misadventure.
• The Patient’s Referrer confirms that there is an identifiable clinical need for a HMR Service.
Information Required for Referral
To ensure a smooth and efficient review process, HMR Australia requires the following information from the referring party:
• The clinical reason for the HMR
• Patient's full name, date of birth, medicare card number, address, phone number
• The GP details, including the provider number
• The current medications list
• The Patient's Medical history
• Recent clinical issues and progress notes
• Recent pathology results
• Vital observations, body weight, height, and BMI
