Home Medicines Review (HMR)

General Practitioners
General Practitioners play a key role in identifying patients in their care who may benefit from a home medicine review. Let your patient know that if they are taking more than five medications, they qualify for a fully bulk-billed, in-home education and medication review session with a pharmacist. During this session, the pharmacist will assess how the medications are stored (whether in the fridge or cupboard) and whether they are taken with food, swallowed whole, or crushed. Additionally, the pharmacist will review any vitamins, supplements, fibres, laxatives, or other over -the-counter products the patient may be using.
